In-network with most UnitedHealthcare, UMR, Cigna, and Blue Cross Blue Shield.
*For Blue Cross Blue Shield plans, there is a $150 out-of-pocket charge.

A $50 travel fee may apply if more than 30 minutes away.

For cash pay options, a superbill can be provided to insurance for possible reimbursement.
